schematic for attaching a dandelion chip?

Does anyone have an F mount diagram that shows the measurements of the mount and where the pins are? my dandelion chip fell off. I am not an expert but I'd like to make a 3d pointed Jig to assist in reattaching it in the measurements of the proper location would help.

edit: also, what glue is best?

I have a spare jig for the Dandelion "somewhere". Will look for it tomorow.

The first pin out of the cluster of four pins should point directly up, see below in red.
