A manhole cover in Beaverton, Oregon, a few blocks from the main Nike campus. No leaves--the sidewalk sweeper must've been a bit ahead of me...

A really small one (10 cm diameter).

Almost invariably they paint arrows across them (e.g your photo, Ashlandish). I understand it as a way to mark the direction for opening, or the prying open point, or the direction of water flow, when not evident on manhole itself.

Wind-decorated manhole

A pomegranate plant was losing its flowers, wind carried them a short distance and they were captured by the edges of the manhole...

From the major Electrical Company in Italy, ENEL. Very nice fisheye effect...

A nice square one...

The hole underneath is round I guess. Otherwise the lid can fall down.

Round and Elliptical

Since the diagonal is 1.4142x the sides, it would be very easy to drop the lid inside the hole. I suspect there's a hinge (see center top) to prevent such an instance. You lift it from the opposite side (see lower L and R)

Two smaller ones, one round and the other elliptical. They are used by the Fire Brigade.

Entrance to a WWII bunker in La Línea de La Concepción. If I´m not mistaken it was a german project to have defenses against Gibraltar. This zone was very interesting at the time with many spies, crossing point from and to the mediterranean and from and to Africa-Europe connection.
